Dr Dwyer said Cauchi was “floridly psychotic” on the day. She said he was “effectively without treatment, and without adequate supervision, for nearly five years”.

The court heard his search history showed he was “deeply fixated on weapons, violence, and mass killing”, reports said.

Update

Coroner Teresa O’Sullivan handed down findings on 5 February 2026. She recommended the Queensland Health Ombudsman review the care given by Cauchi’s former psychiatrist. She said that care was not the major reason for the attack, the ABC reported.
